Lakeview Doodles will guarantee your Puppy until 26 months of age from
all life threatening hereditary defects, including severe hip
dysplasia. If the Puppy is found to have a life threatening hereditary
defect,Lakeview Doodles will provide a replacement puppy from the next available
litter.
A veterinary health record for your puppy has been provided,
specifying a veterinary exam, first set of vaccinations and three separate doses of wormer have been given.
The Buyer has 14 days from the time of receiving the puppy, to have
it examined by a licensed veterinarian of Buyers choice, at Buyers sole
expense. If the Veterinarian determines that the puppy has a life threatening hereditary defect and is unfit for sale, then the Puppy may be
returned immediately to Lakeview Doodles within 24 hours of such
determination. A replacement Puppy will then be provided from the next available litter. A statement setting forth such defect from diagnosing
Veterinarian on the Veterinarian's letterhead must be forwarded to Lakeview
Doodles within the above mentioned time period. All veterinary and
transportation expenses are Buyers obligation. Lakeview Doodles
reserves the right to have the puppy with defect examined by a veterinarian of our choice before a replacement is made.
Conditions of guarantee are as follows:
This guarantee is only applicable to the above-named Buyer, the
original purchaser. If the puppy has been sold, or given away, the health
guarantee becomes void. All veterinary cost's are the owner's expense. If the Puppy has been physically injured or abused in any manner, this
guarantee is void.
If the Puppy is diagnosed by OFA (or other accredited certifier) as
being severely dysplastic on or before 26 months of age, the Buyer must
submit a copy of the OFA report to Lakeview Doodles. A veterinarian cannot make the diagnosis, it can only be made by OFA, Penn Hip, or other
accredited hip specialist certifiers and the Puppy must be sedated for
the radiograph to be taken.
The Puppy must be kept current on all vaccinations including a rabies
vaccination after 4 months of age. The Puppy should be kept on a flea
and tick preventative as well as a monthly worm guard preventative. Copies of these records will be required to be submitted to Lakeview
Doodles if Buyer is requesting a replacement. This health guarantee is void
if the puppy has not been given necessary vaccinations, regular heart
worm, flea and tick preventatives, or is housed in an outdoor kennel.
Dogs must be housed indoors.
The Undersigned Buyer has read the above conditions and agrees to
abide by them.
