Puppy Information: DOB: _____________________ This agreement is between Seller, Elizabeth Engelbert, and Buyer____________________________________. A veterinary health record for your puppy has been provided specifying a veterinary exam, first set of vaccinations and several separate doses of wormer that have been given. Though NYS law regulates that the Buyer has 14 days from the time of receiving the puppy to have it examined by a licensed veterinarian of Buyers choice at Buyers sole expense, Lakeview Doodles requests that an initial exam is done within 3 business days of taking possession of the puppy; with the NYS return policy standing in effect for the 14 days. Even though at Lakeview Doodles we do our best to rid your puppy of worms, it is possible that the puppy may still have worms. You should have your vet do a routine stool sample check at each visit in order to ensure you have effectively treated long enough to break the continuing cycle. We do not guarantee that your puppy will not have parasites, it is possible for them to have a negative stool sample at our vets, then become infected again due to the stress of moving. * Lakeview Doodles guarantees your Puppy from hip dysplasia, elbow dysplasia, eye conditions If the Veterinarian determines that the puppy has a life threatening hereditary defect as stated above, a statement setting forth such defect from diagnosing Veterinarian, on the Veterinarian's letterhead, must be forwarded to Lakeview Doodles within 24 hours of such determination. Lakeview Doodles reserves the right to have the puppy examined by a veterinarian of our choice, at our expense; all other veterinary and transportation expenses are the Buyers obligation. If the puppy is found to have a life threatening hereditary defect as stated above, Lakeview Doodles will reimburse buyer for veterinary expenses related to the defect up to the purchase price of the puppy, with the buyer retaining ownership of the puppy. Shipping fees will not be refunded. This warranty does not include viral illnesses, infections, improper bites, hernias, hypoglycemia, Guardia or Coccidosis. It also does not include any illness due to the ingestion of foreign objects, food, chemicals, physical injury or issues whose cause is unknown or may have an environmental link. _______ int. page 1 of 2
Conditions of guarantee are as follows: This guarantee is non-transferable. If the puppy has been sold, or given away, the health guarantee becomes void. All veterinary cost's are the owner's expense. If the puppy has been physically injured or abused in any manner, this guarantee is void. If the puppy is diagnosed by OFA (or other accredited certifier) as being severely dysplastic on or before 26 months of age, the Buyer must submit a copy of the OFA report to Lakeview Doodles. A veterinarian cannot make the diagnosis, it can only be made by OFA, Penn Hip, or other accredited hip specialist certifiers and the puppy must be sedated for the radiograph to be taken. The puppy must be kept current on all vaccinations including a rabies vaccination after 4 months of age. The puppy should be kept on a flea and tick preventative as well as a monthly heartworm preventative. Copies of these records are required to be submitted to Lakeview Doodles if Buyer is requesting reimbursement. This health guarantee is void if the puppy has not been given necessary vaccinations, regular heart worm, or is housed in an outdoor kennel. Dogs must be housed indoors. If the puppy should die from a suspected congenital defect within 26 months of age an autopsy must be performed, at the Buyers expense, by a state licensed Veterinarian. If the autopsy determines that a congenital defect is the cause of death, Lakeview Doodles will refund the expense of the autopsy as well as provide another puppy of equal value to the Buyer or refund the purchase price of the puppy. All purchase agreements and spay/nueter contracts must be signed and returned to Lakeview Doodles within 14 days of taking possession of the puppy or health guarantee is void. All certificates of spay/nueter must be provided to Lakeview Doodles by the time the puppy is 7 months of age or the health guarantee is void and legal action may be taken at the Buyers expense. Spay & Neuter Agreement Name ________________________________________(Buyer) In consideration for the purchase of the Lakeview Doodle described below: _____________________________________________ Conditions of Sale: ____ (2) Buyer agrees to be responsible for any and all associated veterinary costs, and to provide Seller with a certificate of alteration signed by a licensed veterinarian within 7 months of the puppy's date of birth (__________). ____ (3) If Buyer fails for any reason to spay/neuter the above doodle on or before the date listed and/or provide the Seller a certificate of alteration, the Buyer agrees to pay for any and all fees, including but not limited to, legal fees, court costs, depositions, telephone bills, veterinary expenses, etc. incurred by the Seller and it's agents to enforce this contract. Such failure will also invalidate the health guarantee provided for the doodle. ____ (4) Transfer of ownership or housing of this doodle does not limit or in any way change the responsibility of the above named Buyer to spay/neuter the animal as per this agreement. ____ (5) It is understood that no registration papers will be provided for this doodle until the spay/neuter agreement has been completed.
